What Is a Medicare Annual Wellness Visit?

The Medicare Annual Wellness Visit (AWV) focuses on prevention and health planning rather than a head-to-toe physical examination.

What Is Included in a Medicare Annual Wellness Visit?

Your visit includes a Health Risk Assessment (HRA), which gathers information about your current health, lifestyle, and potential health risks. According to CMS, the HRA is a required part of the Medicare Annual Wellness Visit and helps your provider develop or update your personalized prevention plan.

Review of Your Medical and Family History

Your provider will review or update information about your:

  • Medical conditions

  • Previous illnesses and surgeries

  • Family health history

  • Allergies

  • Medications and supplements

  • Other healthcare providers involved in your care

Routine Health Measurements

During the first Medicare Annual Wellness Visit, routine measurements generally include:

  • Height

  • Weight

  • Body mass index (BMI), or waist circumference when appropriate

  • Blood pressure

Body mass index (BMI) is a screening measure that compares weight with height. The CDC notes that BMI is most useful when considered alongside other health information rather than as a standalone measure of health. 

Subsequent visits include weight and blood pressure measurements, along with other routine measurements considered appropriate based on your health history.

Cognitive Assessment

Your provider will look for signs of possible cognitive impairment, including changes in memory, concentration, decision-making, or the ability to manage everyday activities.

If signs of cognitive impairment are identified, Medicare may cover a separate visit for a more detailed cognitive assessment.

Fall Risk and Functional Ability

Your provider may also discuss:

  • Balance or fall risk

  • Hearing

  • Mobility

  • Home safety

  • Activities of daily living

Is the Medicare Annual Wellness Visit Free?

For eligible people with Medicare Part B, Medicare states that you pay nothing for the yearly Wellness Visit if your doctor or other qualified healthcare provider accepts assignment. The Part B deductible does not apply to the covered Wellness Visit. 

However, that doesn't necessarily mean everything discussed, tested, or treated during the appointment is free.

When Could You Have Additional Costs?

You may have to pay coinsurance, and the Part B deductible may apply, if your provider performs additional tests or services during the appointment that aren't covered under the preventive Wellness Visit benefit.

For example, if you come in for your Annual Wellness Visit but also ask your provider to evaluate a new medical problem, that evaluation may be considered a separate medically necessary service and may involve cost-sharing.

The Annual Wellness Visit itself does not automatically include clinical laboratory testing. Your provider may recommend or order appropriate tests separately.

Before having additional services, ask whether they are part of your covered Wellness Visit and whether you may have an out-of-pocket cost.

A Medicare Annual Wellness Visit is not the same as a routine physical exam. The main difference is the purpose and scope of the visit:

Medicare does not consider the Annual Wellness Visit a routine physical. If you need evaluation or treatment of a specific health concern during the same appointment, additional services may be billed separately and could involve out-of-pocket costs.  

What Other Preventive Services Does Medicare Cover?

The Annual Wellness Visit can help identify which Medicare preventive services may be appropriate for you.

Depending on eligibility requirements, Medicare Part B covers many preventive and screening services, including cardiovascular disease and diabetes screenings, depression screening, certain cancer screenings, bone mass measurements, lung cancer screening, glaucoma screening, and certain vaccines. Other preventive services and vaccines may be covered under different parts of Medicare.

Who Is Eligible for a Medicare Annual Wellness Visit?

Medicare Part B covers a yearly Wellness Visit for eligible beneficiaries.

Your first Annual Wellness Visit cannot take place within 12 months of enrolling in Medicare Part B or within 12 months of receiving your “Welcome to Medicare” preventive visit.

You do not have to complete a Welcome to Medicare visit before becoming eligible for an Annual Wellness Visit. After you're eligible, Medicare generally covers the Wellness Visit once every 12 months.
